When to do the I-485 medical exam?

How early can I do my medical exam? Is it valid for one year after submitting?
I heard lots of people get RFE for medical exam being expired.

to Ramon:

You gotta submit i-485 within 60 days of the civil surgeon signature so you need to count the dates carefully.
